Health Questionnaire

Please download and complete this form before attending your first class. All information will be treated confidentially in accordance with our privacy policy and Isle of Man GDPR.

By signing the PAR-Q document, you acknowledge and agree to the following:

1. Participation & Risk

I understand that participation in classes and activities at The Pilates Boutique, including the use of reformer equipment (“Activities”), involves inherent risks, including but not limited to serious injury, permanent disability, or death, as well as loss or damage to personal property.

2. Assumption of Risk

I voluntarily accept full responsibility for all risks associated with participation. I understand that these activities are physically demanding and that improper use of equipment or failure to follow instructions may increase the risk of injury.

3. Safety & Instruction

I agree to follow all guidance provided, use equipment as directed, and exercise within my own physical limits. I understand failure to do so may result in injury or removal from the session.

4. Nature of Instruction

I acknowledge that all guidance provided is for general fitness purposes only and does not constitute medical advice, diagnosis, or treatment.

5. Reformer Equipment

I understand that reformer equipment carries additional risks. I confirm I will complete a mandatory taster session unless prior experience has been approved.

6. Health Declaration

I confirm that I am physically fit to participate and have disclosed any relevant medical conditions, including (but not limited to): injuries, spinal issues, joint conditions, pregnancy, or cardiovascular concerns.

I confirm I have sought medical advice where necessary.

7. Participation with Medical Conditions

If I choose to participate with any injury, condition, or during pregnancy, I do so entirely at my own risk.

8. Conduct

I agree to follow studio rules, including hygiene standards. I understand the studio is not responsible for lost or stolen belongings.

9. Media Consent

I consent to photographs and videos being taken during sessions for marketing and promotional use unless I notify the studio otherwise in writing.

10. Liability Waiver

To the fullest extent permitted by law, I release and hold harmless The Pilates Boutique, its owners, and representatives from any liability for injury, loss, or damage arising from my participation, except where caused by negligence or where liability cannot be excluded by law.

11. Agreement

I confirm that I have read, understood, and agree to this waiver. This forms a legally binding agreement governed by Isle of Man law.
